Option one treats the attic floor. Every pot light, wire hole and the hatch frame is sealed with foam before depth is added, so the attic itself stays cold and ventilated, as most houses were built.
Option two puts attic spray foam insulation against the roof boards from inside. The entire attic becomes conditioned space, which suits finished attics, cathedral ceilings and attics holding ductwork or a furnace.
A tall pile of loose fibre accomplishes little while warm air streams past it. That air carries moisture too, which shows up as frosted nail tips and brown ceiling stains.

Cambridge roofs carry snow for weeks at a stretch. Heat seeping into the attic melts it from below, the water refreezes at the cold eaves, and ice dams build. Sealed foam holds the heat inside the living space, so the roof surface stays cold and the snow just sits there.
Cathedral and sloped ceilings leave only the depth of the rafter for insulation, often just six or eight inches. Batts squeezed into that space sag away from the roof boards and leave cold strips along the ceiling. Closed cell attic spray foam fills the rafter bay tight to the wood and gives far more R-value in the same depth.
This is common in Fiddlesticks and other newer parts of Cambridge, where vaulted living rooms and bonus rooms sit right under the roof. It also suits older story-and-a-half houses in Hespeler with knee walls and sloped bedroom ceilings.
Ladders, hoses and drop sheets come in through the hatch or the knee wall door. Bathroom fans and pot lights get boxed or sealed first so foam stays where it belongs. Most attics take one day, and the house is aired out before anyone sleeps upstairs again.
